We are looking for a Registered Nurse to provide nursing care to patients in their homes and, on rostered days, at our partner rehabilitation centre. You will support recovery after surgery, hospitalisation, or for chronic conditions, working closely with physiotherapists and the wider allied-health team.
Responsibilities
Conduct home-visit nursing assessments and care (wound care, medication management, vital signs monitoring, catheter/stoma care as required).
Deliver centre-based nursing support on rostered days, working alongside physiotherapists and occupational therapists.
Maintain accurate, timely clinical documentation for every visit, in line with professional and regulatory standards.
Educate patients and caregivers on post-discharge care, medication adherence, and warning signs requiring escalation.
Liaise with referring hospitals, physicians, and family members to coordinate a safe care plan.
Escalate clinical concerns promptly and appropriately, including emergency situations.
Maintain required nursing supplies and equipment for home visits.
Requirements
Registered Nurse with current, valid registration with the Singapore Nursing Board (SNB).
Minimum 2 years of clinical experience in a public healthcare institution (e.g. restructured hospital, polyclinic, or public step-down/community care facility).
Comfortable working independently in patients’ homes as well as in a team-based centre setting.
Strong communication skills with patients, caregivers, and referring clinicians.
Willing and able to travel to patients’ homes across Singapore (own transport preferred).
Basic Cardiac Life Support (BCLS) certification (or willingness to obtain/renew).
Nice to have
Experience in community nursing, home care, or step-down/transitional care.
Wound care or chronic disease management certification.
Experience working with elderly or post-surgical patients.

We are looking for a Registered Physiotherapist to deliver home-based rehabilitation and, on rostered days, centre-based sessions using our partner centre’s equipment (including robotic gait training and pneumatic strength equipment). You will manage a caseload spanning neurological, orthopaedic, and geriatric conditions.
Responsibilities
Assess patients at home and develop individualised rehabilitation programmes for conditions such as stroke, post-surgical recovery (e.g. knee/hip replacement), and general deconditioning.
Deliver centre-based treatment sessions on rostered days, including use of specialised equipment (e.g. Lokomat, AlterG, HUR equipment) where clinically appropriate.
Track and document patient progress against goals, adjusting treatment plans as recovery progresses.
Educate patients and caregivers on home exercise programmes and safe movement practices.
Communicate with referring physicians and the wider care team on patient progress and any clinical concerns.
Maintain accurate, timely clinical documentation in line with AHPC and professional standards.
Requirements
Physiotherapist registered with the Allied Health Professions Council (AHPC), Singapore.
Minimum 2 years of clinical experience in a public healthcare institution (e.g. restructured hospital, polyclinic, or public rehabilitation/step-down facility).
Comfortable managing a varied caseload independently across home and centre settings.
Strong assessment, clinical reasoning, and communication skills.
Willing and able to travel to patients’ homes across Singapore (own transport preferred).
Nice to have
Experience with neurological rehabilitation (stroke, Parkinson’s, spinal cord injury) or orthopaedic post-surgical rehabilitation.
Familiarity with robotic/technology-assisted rehabilitation equipment.
Experience treating geriatric or falls-risk patients.
3 days per week (flexible arrangement)Remote (Singapore-based, teleconsultation), with occasional centre-based sessions at our partner rehabilitation centre
We are looking for a Registered Dietitian to provide remote nutrition assessment and counselling for patients recovering from surgery, managing chronic conditions, or requiring nutritional support alongside physiotherapy and nursing care. Most sessions are conducted via teleconsultation, with occasional in-person sessions at our partner centre.
Responsibilities
Conduct remote nutrition assessments and develop individualised nutrition care plans, primarily via teleconsultation.
Support patients managing conditions such as post-surgical recovery, diabetes, malnutrition, dysphagia, or weight management.
Coordinate with physiotherapists, nurses, and physicians on integrated care plans where nutrition intersects with rehabilitation.
Provide occasional in-person consultations at our partner rehabilitation centre when clinically necessary.
Maintain accurate, timely clinical documentation for every consultation.
Educate patients and caregivers on practical, sustainable dietary changes.
Requirements
Registered Dietitian with current, valid registration with the Singapore Nutrition and Dietetics Association (SNDA) or equivalent recognised body.
Minimum 2 years of clinical experience in a public healthcare institution (e.g. restructured hospital or polyclinic).
Comfortable delivering care remotely via video consultation, with reliable internet access and a private consultation space.
Strong written and verbal communication skills for remote patient education.
Willing to travel occasionally to our partner centre for in-person sessions when required.
Nice to have
Experience in clinical nutrition for post-surgical, geriatric, or chronic disease patients.
Experience with telehealth platforms.
Certification in a nutrition sub-specialty (e.g. renal, oncology, or diabetes nutrition).
